On Book Day, Wednesday, August 15th, a sports medicine team from the Ochsner Medical Center in Kenner will be providing physical examinations from 9:15am-12:30 pm in the Auxiliary gym. The fee is $ 20.00, paid on the day of the physical in cash or by check, made payable to “Mount Carmel Academy”.  This is separate from all other fees.  The LHSAA Medical History Evaluation form must be signed and completed (Parts I, II and III) before the physical can be given.

  • Important: The medical team will leave at 12:30PM—Students must arrange their time (either before or after book day activities) to ensure completion of their physicals before the doctors leave.

 

o                                Students should wear T-shirts and shorts for the physical.
